Error in prove of HTML

I'm getting this error when testing the Capstone Project Part 3 - Opportunity Intake Assessment course. This only happens with the HTML; I was even using the template available from one of the links, but it still fails. When I test it in debug mode, it works without problems and adds the data to Google Sheets. I literally followed the video instructions, but there were no problems in that video. Can you help me?
